Two Rivers Public Health Department conducts the home visitation program through Healthy Families of America. Healthy Families of America is an evidence-based program that supports families as they work toward a singular vision: all children receive nurturing care from their family that leads to a healthy, long, and successful life.
This program is a free and voluntary in-home relationship-based service, designed to promote positive parent-child interactions, healthy attachment, and partner with parents to feel more confident.
We are accepting referrals for families! Please use this referral link to start the process today.
Referrals accepted for:
• People who live in our district (Buffalo, Dawson, Franklin, Gosper, Harlan, Kearney, or Phelps Counties in Nebraska)
• Pregnant mothers, or babies up to 3 months in age
